Circle of Care is comprised of a multi-disciplinary healthcare team including licensed healthcare professionals in the RN/RPN and Social Work fields.  Our Client Services Supervisors (RN/RPN) and Social Workers play an integral role to providing exceptional care to clients as well as support to caregivers and our front-line Personal Support Workers.  As a licensed healthcare professional you can expect opportunities to contribute your skills to the agency and community, work as part of a multi-disciplinary team and progress your knowledge.

Client Services Supervisor (RN/RPN) Role

Work Life Balance

You will have the freedom through flexible scheduling and a hybrid practice setting (community and home-office based time) to manage your schedule to best meet the needs of your clients, Personal Support Worker team and individual wellbeing.  At Circle of Care we value time off for our employees to rest and re-charge.  We support this by including vacation, sick and personal days in your offer of employment.  Your core business hours are day shifts Monday through Friday with occasional on-call and flexible schedule requirements.

Leadership

You will engage, motivate and mentor a large team of Personal Support Workers towards delivering exceptional client care.  Circle of Care provides you an opportunity to work autonomously in the community managing a case load of clients and many important decisions that impact their care.  You will utilize your knowledge and expertise in this independent work setting requiring critical thinking and problem solving capability.  In addition, more senior leadership roles exist in our Client Services team providing potential development opportunity to continue to grow your career.

The Client and Caregiver Partnership

You will have an opportunity to connect with clients in the safety and comfort of their home where partnering on an individual’s health is made most possible.  Setting clients up for success to age independently at home provides a meaningful contribution to the wellness of our communities.  You can take fulfillment from your impact on caregivers who are able to rest more easy knowing their loved one is cared for.  Often simply providing a listening ear is the start to a positive impact in helping navigate healthcare resources for our vulnerable population and caring for the whole person.

Social Worker Role

At Circle of Care, our team of Social Workers collaborate with internal and external partners to provide the best possible support for our clients.  You will work 1:1 with individuals in their homes as well as in group settings within the community.  You will impact client’s short and long term goals through assessment and care planning.  You can take great pride in knowing the supports you provide to those in need benefit not only individual clients, but entire communities.  Navigating community supports requires ingenuity and resourcefulness and you will have an opportunity to work as part of a close knit team to share best practices.
