Community Health Links
Health Links is a new model of care that makes it easier for people who rely on our system the most to access health care in Ontario. Health Links is centered on one simple concept: physicians, specialists, hospitals, home care, long-term care and community services working together with patients, families and caregivers to better coordinate care.

6250 Bathurst Street – Support Housing Program
Circle of Care provides 24-hour personal support services for the City of Toronto at 6250 Bathurst Street to tenants who meet the eligibility criteria.
Circle of Care is a partner in the Ontario Caregiver Coalition (OCC). The OCC’s focus is to advance the interests of caregivers in the province. It is a group of diverse organizations that work to raise the profile of caregivers and to promote public policy to support caregivers. Membership includes those from health charities, unions, academia, home health and community support agencies, and informal caregivers.

Association of Jewish Seniors
Founded in 1970, the Association of Jewish Seniors (AJS) draws together seniors, organizations, and clubs to educate and increase awareness about health, social, economic, and recreational issues affecting the well being and lifestyles of seniors.
The Bathurst-Finch Community Food Bank is funded by the Ontario Trillium Foundation in collaboration with Unison Health & Community Services, FoodShare Toronto and Circle of Care. The community food bank services are part of a larger project to connect food-related programming across the neighbourhood.
Circle of Care is proud to be a supporting partner in the Bathurst-Finch Hub, which recently opened its doors! The brand new Hub is located next to Northview Heights Secondary School near the corner of Bathurst and Finch and offers community and medical services, all in one building.
The first floor houses community agencies offering services such as counselling, employment services, seniors’ activities and youth groups. There is also space that community groups can use including a kitchen and meeting rooms. On the second floor, there is a health clinic with family doctors, counsellor, dietitians, chiropodists and health promotion staff from Unison Health and Community Services.
In 2014, Circle of Care was the proud recipient of a donation of bottled water by Project Water. This charity was founded 15 years ago in response to the dehydration crisis that many people on the streets face during the hot summer months. The bottles of water donated to Circle of Care will be handed out to isolated seniors and transportation clients during heat waves in the summer and emergency situations in the winter.
