Home Support Services

From meal preparation and light housework to personal care and more, our Home Support Services help individuals and families manage the tasks of everyday living.

Whether you need temporary relief from caregiving or household routines, or you require major assistance with complex and demanding needs, our specially trained Personal Support Workers can help.

Participate in personalizing your care plan
Needs and circumstances vary from family to family. Our Home Support Coordinators can help assess the type of support and the level of assistance needed to ensure safety, comfort, and independence.

Where your family is eligible for publicly funded home support, our staff will ensure all referrals and coordination of services are met.


Specially trained Personal Support Workers
All our Personal Support Workers have received extensive training and certification. They are supervised regularly, and provided with annual in-service training.

Familiarity is important. After you choose the service times convenient for you, we strive to maintain the same schedule and worker for your family whenever possible.


Kosher Meals on Wheels

Good food provides more than simple nourishment.

It provides pleasure and comfort, energizes the body, celebrates traditions and values and reinforces important bonds with others.

Kosher Meals on Wheels delivers food every weekday, representing a safe, consistent, and positive link to heritage and the outside world.

All meals are prepared under the supervision of the Kashruth Council of Toronto. They also meet national nutritional standards as planned by qualified dieticians.

Menus for different diets are available, including ones for: diabetics; those needing minced, soft or pureed foods, no added salt foods, or low fat foods; or for people who are lactose intolerant.

Each meal includes a vegetable, a dairy or meat entrée, soup, juice, bread, and a dessert.

Weekday meals are delivered by Circle of Care volunteers by noon, Monday to Friday.

Frozen entrées and soups are available for weekends or holidays.

Meal delivery can be scheduled for any number of days, from only a few days to an indefinite period of time. Cancellation of meal delivery must be made at least 24 hours beforehand.


New Snack Program

Delicious and nutritious snacks are now available to Kosher Meals and Wheels recipients. Fresh and appetizing selections of cold foods are available at a nominal fee along with regular delivery to provide a second light meal to help maintain a balanced and healthy diet.

Cost

Kosher Meals on Wheels and Snacks are affordably priced, and subsidies are available to eligible persons.

Circle of Care volunteers provide door-to-door delivery and help organize all aspects of the Kosher Meals on Wheels program. Volunteer today to support independent living.

Circle of Care staff are in regular contact with each recipient of Kosher Meals on Wheels and the new Snack Program to ensure satisfaction and that dietary needs are met.

Social Work Services

In coping with significant health changes – whether it's you or a loved one – it helps to reach out to others.

Some want to talk about their concerns and feelings. Some need to learn new ways of caring for themselves, their loved ones, or their households. And others simply need general guidance in coping with major change.

At Circle of Care, we understand this. That's why we offer a wide range of Social Work Services led by a team of experienced, professionals. Our clients can select what they need to cope with change, in ways best suited to their preferences and circumstances.


Circle of Care Social Work Services are designed for individuals who are:

• Experiencing change in memory functions

• At risk of physical, verbal, financial, alcohol or drug abuse, or of neglect

• Feeling the emotional and physical strain of caring for another

• Dealing with mental health issues

• Facing important decisions about long-term care or housing

• Vulnerable and isolated without anyone to support them.


From your first call to Circle of Care, you will be assured of our respect, confidentiality, and professionalism. Our Intake Worker will help you identify your needs. From there, you may be referred to a Social Worker who can help you:

• Assess your ability to function independently in your home

• Develop a care plan with you, family members, and your health care professionals

• Coordinate the services and resources of other Circle of Care departments and other agencies

• Monitor your situation through home visits, telephone conversations, and office interviews

• Invite you to participate in a Circle of Care Support Group

• Assist you in completing forms for short and long-term care and housing

• Provide individual and family counselling or support.

Our Social Workers

Circle of Care Social Workers must have a degree in Social Work from an accredited university, be eligible for accreditation by the Ontario College of Social Work and have at least 5 years of experience in working with individuals and families.

Adult Sons and Daughters Support Group
For mutual support and education of adult children who are caring for a parent or in-law with Alzheimer Disease Disease or a related disorder.

Early Stage Dementia Support Group
For mutual support and education of individuals who are in the early stages of Alzheimer Disease or a related disorder.

Support Group for Abused Older Women
For mutual support and education of women over age 55 who have been or are currently in abusive marital relationships.

Bereavement Support Group
For mutual support and education of individuals who have suffered a loss.
