Most people wish to spend their final days in the comfort of their home. However, the logistics of arranging for end-of-life care at home can be overwhelming. We have specially trained staff and volunteers and a range of services to help make this possible. Our services include:

End-of-life in-home support
We offer:

  • Visiting Hospice Program
  • Personal care (e.g., grooming, bathing, dressing)
  • Homemaking, including shopping, meal preparation and light cleaning
  • Caregiver relief
  • Subsidies available (conditions apply)

Visiting Hospice
Circle of Care has visiting hospice services that respond to diverse needs, including a specific program designed for the Jewish community (Jewish Hospice Program), and a non-denominational program. Our program is accredited by Hospice Palliative Care Ontario. Circle of Care’s hospice volunteers are carefully screened, receive over 30 hours of training and are supervised by our Hospice Case Manager. These dedicated volunteers are available 2 to 4 hours a week to provide emotional support, companionship, caregiver relief and practical assistance.

With additional training, hospice volunteers become equipped to extend their involvement with the caregiver or family they have been visiting. Bereavement support provided by volunteers helps families learn to adjust to life without their loved one.

The Hospice Case Manager is available for in-home visits and can assist difficult discussions around end-of-life issues, anticipatory grief, family communication and decision making and comfort care. Cultural sensitivity is practised. The Hospice Case Manager also provides after-care in the form of more intensive bereavement support and counselling and to suggest appropriate community resources.

The Jewish Hospice Program is a partnership with Jewish Family & Child, Mount Sinai Hospital (Tammy Latner Centre for Palliative Care), Baycrest, and UJA Federation of Greater Toronto.
